This application form allows you to save and resume your application later. To do this, click the save and resume later button at the end of the page. This will provide you with a link to your form. Please copy and save this link. You should also select to receive an email of the link. You cannot resume your application without the link, so you’ll have to start from scratch!

Currently, the USBG Foundation only accepts applications for the Helen David Relief Fund, which helps bar workers with cancer-related hardships. 

Please visit our resources page for other assistance platforms if your hardship is not cancer-related. The Bartender Emergency Assistance Program will be reinstated when additional funding is secured.
